Before the trip to Russia/Ukraine:

1. Print and sign Contact List, collect  adoption medical information, enclose a check for $300 payable to the IAMC and mail it to IAMC, 515 Washington St. #10I, Vancouver, WA 98660.

2. Dr. Benikova will review the information, and will be available to answer your questions over the phone or e-mail.

3. For your trip, print and bring the Intake Questionnaire (in Russian). Present this Questionnaire to the orphanage pediatrician and ask her/him to fill it out. The Intake Questionnaire is designed to streamline the evaluation process, but is not mandatory. For your reference you may print the English copy of the Intake Questionnaire.

On the Trip:

4. Dr. Benikova is available to answer your medical questions in real time over the phone, e-mail or fax.

5. Collect medical records and all related medical and social information that is normally provided by your adoption agency. Remember that you do not need an English translation of medical records. In fact, Dr. Benikova would prefer to have a copy of original medical records in Russian or Ukrainian. If the original medical records are not available, please ask your agency to provide them.

6. As soon as the medical records are available to you, fax or e-mail them to our office along with contact phone numbers for the orphanage pediatrician and for you. Dr. Benikova will review records, talk to the orphanage pediatrician (if necessary), and provide you with a medical assessment over the phone, e-mail or fax.

After the trip:

7. Note that any additional medical information that was not available at the time of evaluation will be reviewed free of charge.

You can also fax your documents to 503-977-0916 or e-mail them, but they will not be evaluated until we get the payment.

6. You can expect the evaluation report to be e-mailed to you within 72 hours after we received your documents and payment. Our Office Manager will contact you to schedule the Review and Q&A session over the phone with Dr. Benikova.

7. Note that any additional medical information that was not available at the time of evaluation will be reviewed free of charge.
